Through Medicare Connected Care, you can receive direct support for your chronic conditions from a personal care manager — even when you’re not in your doctor’s office. Phamily works directly with medical practices to provide the most convenient and high-quality patient care possible.

Covered by Medicare & more. Connected Care is covered by Medicare and some Medicaid and commercial insurances. Standard cost sharing rules apply, but many patients with Medigap or Medicaid have no out-of-pocket costs.*
